Orthopedic Aftercare ICD-10 Codes: Z47.1, Z47.89 and the Rest of Z47 (2026)
By ClinicFlow Team
The ICD-10 code for orthopedic aftercare comes from category Z47. Z47.89, encounter for other orthopedic aftercare, is the general code, but use a more specific one when it fits: Z47.1 for aftercare after a joint replacement, Z47.2 for removal of an internal fixation device, Z47.31-Z47.33 after a shoulder, hip or knee prosthesis is removed, Z47.81 after an amputation and Z47.82 after scoliosis surgery. Z47 is not used for injuries: aftercare for a fracture is coded with the injury code and the 7th character D.
For practice managers: aftercare is where post-op patients call the most. To hear an AI receptionist book a post-op visit and route an urgent post-op call, dial our demo line at (415) 962-4019, or book a demo.
Every Z47 code
| ICD-10-CM | Description | Typical use |
|---|---|---|
| Z47.1 | Aftercare following joint replacement surgery | Visits after a knee, hip or shoulder replacement |
| Z47.2 | Encounter for removal of internal fixation device | Plates, screws, rods or pins being taken out |
| Z47.31 | Aftercare following explantation of shoulder joint prosthesis | Shoulder prosthesis removed, for example in a staged revision |
| Z47.32 | Aftercare following explantation of hip joint prosthesis | Hip prosthesis removed |
| Z47.33 | Aftercare following explantation of knee joint prosthesis | Knee prosthesis removed |
| Z47.81 | Encounter for orthopedic aftercare following surgical amputation | Aftercare after an amputation |
| Z47.82 | Encounter for orthopedic aftercare following scoliosis surgery | Aftercare after scoliosis surgery |
| Z47.89 | Encounter for other orthopedic aftercare | Orthopedic aftercare no other Z47 code describes |
What to pair them with
Z47.1 needs the joint. Report it with a Z96.6- code that names the artificial joint and the side:
| ICD-10-CM | Description |
|---|---|
| Z96.651 / Z96.652 / Z96.653 | Presence of right / left / bilateral artificial knee joint |
| Z96.641 / Z96.642 / Z96.643 | Presence of right / left / bilateral artificial hip joint |
| Z96.611 / Z96.612 | Presence of right / left artificial shoulder joint |
Z47.89 after spine surgery. Aftercare following a spinal fusion that isn't for scoliosis is usually Z47.89 with Z98.1 (arthrodesis status).
For the surgery itself, see knee replacement CPT codes and our orthopedic CPT codes list. For diagnosis codes by body region, see ICD-10 orthopedic codes.
When not to use Z47
- Injuries. The ICD-10-CM Official Guidelines say aftercare Z codes are not used for aftercare of injuries. A fracture follow-up gets the fracture code with the 7th character D (subsequent encounter), not Z47.89.
- The global period. A routine post-op visit inside a procedure's global period (90 days for most major orthopedic surgery) is part of the surgical payment and isn't billed separately, whatever diagnosis is on it. Our global periods guide explains what's in the package.
- A complication. If the visit is for a complication of the surgery, the complication is coded, not routine aftercare.
The phone side of aftercare
I'm a spine surgeon in Houston and co-founder of ClinicFlow. ClinicFlow doesn't code or bill. It answers the phone, and aftercare is a phone-heavy part of the practice: patients move post-op visits, ask what to expect, and call at night when a wound or a fever worries them.
- Post-op visits get booked. ClinicFlow books, reschedules and cancels directly in your EHR (Epic, athenahealth, eClinicalWorks, ModMed, AdvancedMD and other EHRs through HL7), following your providers' rules.
- Urgent calls after hours reach the surgeon on call by text or call, with a summary of the conversation.
- Emergencies go to 911. ClinicFlow never transfers an emergency call. It tells the caller to hang up and call 911 now.
- Your practice chooses which calls the AI takes. Our after-hours triage protocol template is a good place to start.
See it with your own post-op calls: book a demo and we'll show a post-op visit rescheduled in your EHR and an urgent after-hours call reaching the surgeon on call.
